About IHD Care

A family-centred approach to supported living and domiciliary care.

IHD Care was built on a simple belief: every adult deserves to live with dignity, purpose, and the right level of support. We run supported living homes and provide home care for adults with mental health needs across the UK.

Our approach is hands-on and relationship-based. Staff stay consistent. Care plans are written with the person, not just about them. And we work closely with CMHTs, social workers, and families every step of the way.

Supported Living

Five homes in Berkshire for adults with mental health needs. Each resident has one consistent key worker and a care plan written around their actual goals — not what a standard pathway assumes those should be.

Domiciliary Care
In your home +

Domiciliary Care

We send support workers to you at home, around your routine. That might be a couple of hours a week or several visits a day — whatever the assessment says is right.

Intensive Support
Intensive +

Intensive Support

For people whose needs are complex or unpredictable, we provide 1:1 or 2:1 support around the clock. Every package is built with the clinical team and reviewed regularly — the aim is always to reduce input over time.

Daily Living Skills

Leaving hospital often means relearning basics — cooking, budgeting, keeping a home. These are the things independent living depends on, and gaps in them are one of the main reasons people struggle once they move on.

Social Skills

Social Skills

Isolation makes recovery harder. We work with people to rebuild confidence in social situations, get back into the community, and build relationships that hold up over time.

Employment Services
Purpose +

Employment Services

Work gives structure. For a lot of people in recovery, it also gives something to get up for. We help people work out what they want, get ready for it, and take the first steps — paid work, volunteering, or a course.

Floating Support
Flexible +

Floating Support

Some people are largely managing but want a regular check-in. We visit at home on a planned basis, catching small problems before they become crises. Hours adjust as things change.

Forensic Placements
Specialist +

Forensic Placements

We take referrals for people with forensic histories stepping down from secure settings. Reporting structures with probation, MAPPA and forensic mental health teams are agreed before anyone moves in.

Personal Budget Support

Personal Budget Support

Direct payments and personal budgets give people real say over their care — but the paperwork can be a lot. We help set things up, explain the options plainly, and make sure the funding goes toward support that actually fits.

Person Centred Solutions

Person Centred Solutions

Some people’s needs don’t fit a standard package. We design support from scratch for those cases — working with the person, their family and any professionals involved to put something together that actually makes sense.

Referrals +

Client Referral

Referrals come from CMHTs, hospital teams, social services, GPs, probation and families. We reply within 24 hours. There’s no long form to fill in before you can speak to someone — call us and we’ll talk you through it.

How it works

From referral to move-in

Submit a referral

Use our online form or call us. We acknowledge all referrals within 24 hours.

Joint assessment

We meet with you and the person concerned to understand their needs and circumstances.

Visit the home

The individual visits the proposed placement and meets the staff and other residents.

4

Supported move-in

Once funding is confirmed, we arrange a smooth transition with the individual and their support network.
